Please check all the information on this form is correct before you send it to us. We will confirm your order with you with a full price quotation. Payment can be made by credit card (Visa, Master Card, American Express, and Discover Card), cashier's check or money order. If you have a picture of a suit you would like made, please scan the picture and email it, along with any special instructions.

If you are a first time customer and are uncomfortable with providing us the measurements as above, you can mail us a sample suit which fits the way you like. We will use this as a model for your new suit and return it to you with your order. We recommend you use express mail since regular mail will take up to three weeks to arrive
